According to a 2008 scientific study published in Neurology, taking Topamax significantly increases the risk of cleft lip and palate birth defects.  The published study reviewed 203 pregnancies.  Overall, the rate of oral clefts observed was 11 times more prevalent than the normal rate.  Due to these and other serious side effects, the United States Food and Drug Administration (FDA) issued a warning for patients taking Topamax.  The warning was raised to Pregnancy Category D, which means there is positive evidence of human fetal risk.

Cleft lip and cleft palate birth defects occur when parts of the lip or palate fail to completely fuse together.  The defect can result in a small, notched lip or in extreme cases, an open groove that extends from the roof of the mouth to the nose.  Cleft lip and palate birth defects can cause problems eating and communicating, and can increase the risk of ear infections.  Most oral cleft deformities can be corrected with surgery.

Other Side Effects of Topamax

Common side effects of Topamax include numbness and tingling (paresthesia), upper respiratory tract infection, diarrhea, nausea, loss of appetite, increased risk of kidney stones, and memory problems.  Other side effects include psychomotor slowing, fatigue, confusion, and suicidal tendencies.  Finally Topamax can decrease the effectiveness of oral contraceptives.

Additional side effects of Topamax include difficulty concentrating, speech problems, lack of coordination, nervousness, aggressive behavior, irritability, mood swings, depression, headache, drowsiness, weakness, excessive movement, uncontrollable shaking, extreme thirst, uncontrolled eye movements, weight  loss, constipation, gas, heartburn, inability to taste food, swelling of the tongue, overgrowth of the gums, dry mouth, increased saliva, trouble swallowing, nosebleed, teary or dry eyes, back, muscle or bone pain, missed menstrual periods, excessive menstrual bleeding, skin problems or changes in skin color, dandruff, hair loss, growth of hair in unusual places, ringing in the ears, difficulty falling or staying asleep, swelling of the hands, arms, feet, ankles or lower legs, and difficulty or pain when urinating.
